Primary Care Nurse Practitioner or Physician Assistant opportunity in Auburn, NY

Great opportunity for a full-time nurse practitioner (NP) or physician assistant (PA) to join our team, providing primary care services in a skilled nursing facility located in Auburn, New York.

Position Highlights:

• Providing comprehensive primary care services to a manageable patient panel of 15-18 patients per day
• Collaborating with interdisciplinary teams to optimize patient outcomes
• Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
