TITLE: Hospice On Call RN
SUMMARY:
The Hospice On-Call Field RN is responsible for providing emergent and urgent nursing care to hospice patients during after-hours periods. This role includes assessing patient conditions, implementing care plans, and coordinating with the interdisciplinary team to ensure continuity of care and adherence to the patient’s goals of care.

QUALIFICATIONS: . RN, 3 Yrs Nursing experience including hospice, medical surgical, or community home health, Must have Valid DL and Car BSN Preferred
Minimum three (3) years of nursing experience, including hospice, medical-surgical, or community home health.Current NYS Registered Nurse license.
Strong clinical expertise in end-of-life care and symptom management.
Excellent communication and organizational skills.
Proficient in using EMR systems and basic computer applications.
Has an ability to work flexibly and handle sensitive situations compassionately.
Valid NYS driver’s license and reliable transportation.
Ability to work non-judgmentally with diverse patient populations.
Education:
Graduate from an accredited school of nursing; BSN preferred.
